fix(verifier): report headings the ship gate reads - #31

roles/verifier.md told reviewers to head each lens `## Verifier report — E2E`, but approve.sh ship reads each VERDICT only under `### E2E`, `### Side effects` and `### Intent match`. A report preserved unchanged, as the role asks, was refused at ship; any `###` sub-heading inside a lens hid its VERDICT too. Seen in a measured compact run: the refusal and the hunt through gate source fell in the 330-420 s evidence/ship span of a 462 s loop. The report template now uses the gate's headings, with `####` for sub-headings. The provenance sentence points at the template lines that already list each item, keeping verifier.md within its word budget (2,422 -> 2,420).

## Verifier report — E2Eheadings, with###sub-headings inside each lens. The first ship approval was refused, and the agent searched gate source twice before restructuring evidence.md (the run's own evidence.md records "the first approval refused alternate reviewer headings"). That rework falls inside the 330–420 s span. This is one run; the share of time the fix saves is not measured.Change
