Adds `Axon.elem/3` for pulling an element out of a tuple-valued container layer and `Axon.fetch/3` for pulling a value out of a map-valued one. Both are sugar over `Axon.nx/3` and give multi-output models a named op in the graph rather than an opaque `:nx` node. Defining `elem/3` with a default argument also defines `elem/2`, which shadows the auto-imported `Kernel.elem/2` inside this module, so Axon now imports Kernel except that function and calls `Kernel.elem/2` explicitly at its existing call sites.
