Add an Issue Template to Update a Project Description

Fixes # - Not applicable. This is a hot fix to add an issue template.

What changes did you make?

• I added "Project Profile description updates" issue template.

Why did you make the changes (we will use this info to test)?

• The issue template was added because the Website Team needs a template to make changes to project descriptions.

CodeQL Alerts

After the PR has been submitted and the resulting GitHub actions/checks have been completed, developers should check the PR for CodeQL alert annotations.
